Offer e-TPE to a patient
Therapeutic Patient Education (TPE) is a personalised support process designed to help patients better understand their condition, manage their treatment, and adopt health-promoting behaviours. At Nantes University Hospital, patients can participate in the TPE programme for hypoparathyroidism. This programme is based on active exchanges via videoconference between patients, the healthcare team, and patient experts from the Hypoparathyroidism France association, aiming to develop self-management skills and improve quality of life. TPE includes activities such as providing information about the condition, managing symptoms, adjusting treatments, and preventing complications. This approach empowers patients to take an active role in their own health.
« Tu m’prêtes ton spé »
Finding specialists for every rare disease is not easy everywhere!
To improve care for patients with rare diseases, our project « Tu m’prêtes ton spé? » allows an expert doctor to visit a centre, share their expertise, and train the local team.
This involves a day of joint consultations between the local referring physician (regardless of the type of healthcare facility) and the visiting expert.
The expert will also deliver an on-site lecture to train healthcare professionals in the hosting centre and surrounding areas. Through this project, we also aim to connect patients with patient associations to help reduce isolation.
The hosting centre incurs no costs, thanks to sponsorship from the Groupama Foundation for Health and Groupama Loire Bretagne. Feel free to contact us for more information about the project, whether as a hosting team or an expert practitioner.
